Position SummaryThe Social Services Director is responsible for planning, organizing, and directing the overall operation of the Social Services Department in accordance with federal, state, and local standards, as well as Althea Woodland’s policies and procedures. This role ensures that residents and families receive the highest quality psychosocial support, discharge planning, and assistance navigating healthcare transitions.
Primary ResponsibilitiesProvide leadership and direction for all aspects of the social services program.
Conduct and document psychosocial assessments upon admission, quarterly, and as needed.
Coordinate discharge planning, ensuring safe and appropriate transitions of care.
Support residents and families through the adjustment process to facility life, maintaining open communication and compassionate advocacy.
Collaborate with nursing, therapy, activities, and administration to ensure a holistic approach to resident well-being.
Participate in interdisciplinary care planning and QAPI activities.
Maintain regulatory compliance with COMAR and CMS standards related to residents’ rights, abuse prevention, and grievance handling.
Supervise and mentor social services staff and assist in staff development and in-service education.
Assist and direct in the admissions and discharge process as needed
Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited institution required.
Current Maryland licensure or eligibility required (LBSW, LMSW, or LCSW).
Prior experience in long-term care or subacute rehabilitation strongly preferred.
Excellent communication, documentation, and organizational skills.
Strong interpersonal skills and the ability to build trusting relationships with residents, families, and coworkers.
