What are the responsibilities and job description for the COMMUNITY BASED CASE MANAGER position at Allwell Behavorial Health Services?
Located primarily in the school, the Community Based Case Manager is responsible for supporting youth and families in homes, schools, our offices and other community settings. Will provide competent support and integrated care to improve youth/family functioning and dynamics. The role includes coordination with schools, courts, social services, and behavioral health systems through contract and billable therapeutic services.
- Minimum of Associates degree in mental health or social work assistant with three years relevant experience.
- Ability to work nontraditional schedule- evenings, weekends, holidays, and be flexible;
- Meet and maintain ABHS vehicle insurance standards;
- Valid Ohio Driver’s license.
- Must possess good organizational and communication skills;
- Must demonstrate good judgment skills;
- Able to utilize basic problem-solving skills;
- Able to resolve crisis situations in the most clinically effective manner;
- Able to use electronic medical record/data management systems.
