What are the responsibilities and job description for the COMMUNITY BASED CASE MANAGER position at Allwell Behavioral Health Services?
Located primarily in the school, the Community Based Case Manager is responsible for supporting youth and families in homes, schools, our offices and other community settings. Will provide competent support and integrated care to improve youth/family functioning and dynamics. The role includes coordination with schools, courts, social services, and behavioral health systems through contract and billable therapeutic services.
