About the Role
As a Partner in Wellbeing, the RN Health Coach provides ‘whole person’ care to address acute and chronic medical conditions. Compile and evaluate medical history and impact on behavioral health diagnosis.
Essential Responsibilities
- Completes client screening and assessment related to overall physical health and the impact on the management of behavioral health concerns.
- Provides short-term medical case management for acute client conditions.
- Collaborates with other Alluma providers to ensure the safe delivery and monitoring of medication plans.
- Provides direct care services as a community provider. Collaborates with other community providers.
- Ensure that physical health care coordination is included in the person-centred plan, through collaboration with the client’s primary care provider and other specialty services.
- Follows protocols for transition to/from inpatient-client psychiatric care, ambulatory and medical detoxifications and step-down service, emergency or urgent care, and residential treatment through utilizing the EAS systems.
- Focuses on integrated health (physical, mental, substance use, and oral) care, ensuring that clients are referred to any necessary internal and/or external services to support their overall well-being.
- Ability to consult, teach, and individualize treatment plans for specific cases and problems.
- Assist with whole health care- Prevention/Screening/Follow-up
- Assist with withdrawal management, coordinating with the medication provider.
- Work with the care coordination team to identify clients who are admitted /discharged from inpatient hospitalization to assure smooth community integration.
Required Education, Certification, and Experience
- Current MN and ND Registered Nurse License.
- Two plus years of experience in direct nursing, either clinic or community-based; psychiatric experience, preferred
- CPR certified.
- Must pass a criminal background check through MN DHS NetStudy.
- Access to a personal vehicle and the ability and willingness to use the vehicle in carrying out position responsibilities.
- Valid driver’s license and insurable under the agency’s automobile insurance policy.
- Free from substance use problems for no less than 2 years.
Salary : $72,000 - $85,000
