Job Overview
We are seeking dedicated and skilled Palliative Care NP's to join our healthcare team. The ideal candidates will possess a strong background in Palliative Care and demonstrate expertise in patient care, clinical research, and the treatment of chronic or life limiting illnesses.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Manage and administer medications for pain and symptom relief, including both pharmacological and non-pharmacological interventions.
- Collaborate with multidisciplinary teams, including physicians, social workers, and other specialists, to ensure coordinated care.
- Facilitate advance care planning and discussions about goals of care with patients and families.
- Monitor and manage complex symptoms, escalating care to physicians or other healthcare professionals when necessary.
- Provide education and guidance to patients, families, and staff on palliative care principles and best practices.
- Maintain accurate documentation and comply with State, federal and facility policies, infection control, and quality improvement initiatives.
- Collaboration: Ability to work closely with other healthcare professionals, including primary care physicians and specialists, to ensure comprehensive patient care
Requirements:
- Advanced nursing degree (Master’s or higher) and NP certification.
- Previous Clinical experience in palliative care, hospice, oncology, geriatrics, or related fields.
- Strong understanding of medical terminology, coding (CPT, ICD), and EHR documentation practices.
- Experience with critical care, including ICU experience, is preferred.
- Knowledge of related specialties such as endocrinology, cardiology, gastroenterology, and urology is advantageous.
- Excellent communication skills for effective patient interaction and collaboration with healthcare teams.
- Ability to perform basic life support and manage acute pain effectively.
