Work Setting: Home Health – Travel to patient homes required

Alliance Homecare VNA is committed to providing compassionate, dependable home health services to patients in their homes. We are looking for a caring and reliable Licensed Practical Nurse (LPN) to support patient care and work collaboratively with our healthcare team.

• Provide nursing care within the scope of the LPN license and applicable Massachusetts regulations.
• Monitor patients' conditions and report changes promptly to the supervising nurse or appropriate healthcare professional.
• Assist with implementing individualized care plans and physician orders.
• Administer medications and treatments as authorized and within the LPN scope of practice.
• Observe and document patient symptoms, progress, and responses to care.
• Educate patients and caregivers on assigned aspects of their care.
• Maintain accurate and timely clinical records.
• Communicate with supervising nurses and other healthcare team members regarding patient needs.
• Follow infection-control procedures, patient privacy requirements, and applicable clinical guidelines.

• Current, active Massachusetts Licensed Practical Nurse (LPN) license.
• Relevant nursing experience preferred; home health experience is an advantage.
• Strong communication, observation, organization, and documentation skills.
• Ability to follow care plans and work collaboratively with the clinical team.
• Reliable transportation and willingness to travel to assigned patient homes within the designated service area.
• Compassionate, dependable, and patient-focused approach to care.

This position serves assigned patients within our approved service area, which may include Fitchburg, Gardner, Leominster, Lunenburg, Westminster, Hubbardston, Princeton, Sterling, Holden, Worcester, Lancaster, Clinton, Bolton, Berlin, Boylston, West Boylston, Northborough, Shrewsbury, and other nearby communities within the designated area.

Specific assignments and travel expectations will be discussed during the hiring process.

• $38–$48 per hour, depending on experience and qualifications.
