Position Summary

The Social Worker provides resources and support for the Office of Family and Child Health, including Fetal Infant Mortality Review (FIMR), Child death review (CDR), and other related programs. This position plans care, advocates for clients, links clients to community resources, and coordinates services with health care providers and organizations. Responsibilities include documenting client progress, providing home visits, facilitating support groups, and educating clients and families.

Duties

  • Link clients with community resources and services, and facilitate collaboration among healthcare providers, schools, and other organizations.
  • Work collaboratively with other service providers to coordinate care and ensure needed services and supports are provided.
  • Provide advocacy for clients to address systemic barriers to care.
  • Maintain client records, documenting accurately the services provided and the outcomes achieved.
  • Conduct home visits and fieldwork as needed to support client needs.
  • Facilitate support groups and community programs to provide additional support and resources.
  • Educate clients and families about available resources and support systems.
  • Participate in continuing education and professional development opportunities to stay current with best practices in the field.
  • Connect with the community by presenting findings and delivering information to enhance understanding and raise awareness about various program initiatives.
  • Responds to public health emergencies, as assigned
  • Performs related work, as required

Residency: Allegheny County within 1 year.
