- Link clients with community resources and services, and facilitate collaboration among healthcare providers, schools, and other organizations.
- Work collaboratively with other service providers to coordinate care and ensure needed services and supports are provided.
- Provide advocacy for clients to address systemic barriers to care.
- Maintain client records, documenting accurately the services provided and the outcomes achieved.
- Conduct home visits and fieldwork as needed to support client needs.
- Facilitate support groups and community programs to provide additional support and resources.
- Educate clients and families about available resources and support systems.
- Participate in continuing education and professional development opportunities to stay current with best practices in the field.
- Connect with the community by presenting findings and delivering information to enhance understanding and raise awareness about various program initiatives.
