Summary This position supports coordinated care members in accessing physical, oral, and behavioral health services by building trusting relationships and serving as a liaison between the member, service providers, and the community. Approximately 80% of engagement occurs via phone and email, with 20% involving in-person support. The role enhances service quality and cultural competence while increasing health literacy, knowledge, and self-sufficiency through outreach, education, informal counseling, social support, and advocacy.
Essential Duties

Job Duties
- Assists members and their families in accessing community-based support systems, understanding treatment options, and preventive behaviors.
- Helps coordinate member transportation and accompaniment as needed to scheduled appointments.
- Meets members where they are at, in multiple care settings, as needed to ensure that members are supported in making high value care decisions.
- Anticipates, identifies, and helps members to overcome barriers within the health care system to ensure the prompt resolution of emerging health crises.
- Supports and educates members in social support services, basic health education, provides advocacy, health system navigation and community coordination.
- Participates in regularly scheduled staff development trainings to maintain and improve personal knowledge base of various chronic diseases.
- Participates in and attends regularly scheduled supervision meetings and shares both successes and challenges of working with high-need members and their medical team.
- Proactively communicates all concerns to health care team, PCP and program manager, as necessary, seeking support and guidance.
- Interprets and represents the goals and objectives of the community resource specialist to community members and other agencies.
- Assists in quality initiatives for the department, as needed.
- Coordinates care needs for the Dual Special Needs Population (DSNP) member to ensure compliance with the Special Needs Model of Care (SNP MOC).
- Prepares for and participates in all required DSNP training, education, and member audits.
- Participates in initial, annual, and ongoing trainings.
- Acts as preceptor during the onboarding of new hire employees.
- Maintains punctual, regular, and predictable attendance.
- Works collaboratively in a team environment with a spirit of cooperation.
- Respectfully takes direction from care coordination leaders.
- Meets all required training including those listed in Relias Learning Module System (LMS).
- Other duties as assigned.
On Call Responsibilities
This position may have voluntary on- call responsibilities.
Supervisory Responsibilities
This position does not have any supervisory responsibilities.
Job Requirements
Requires the use of personal vehicle for local travel (subject to mileage reimbursement).

Education
High school diploma or general education degree (GED) required. College degree is preferred.
Experience
- At least three to six months’ related experience and/or training in a similar role required.
Certificates, Licenses and/or Registrations
- Valid Oregon Driver’s License and vehicle insurance.
- Current Mental Health First Aid Certification or willingness to be certified within 90 days of hire.
- Current Verbal De-escalation (CPI) Certification or willingness to be certified within 90 days of hire.
- Traditional Health Worker (THW) certification through OHA is required or to be obtained within two years of hire date.
- Cost of certification is company paid and is subject to pay increase upon completion.
- Completes the National Provider Identifier (NPI) registration within the first 30 days of completion of the THW.
Technical Skills
- Exceptional writing, editing, and proofreading skills.
- Excellent computer skills, including the Microsoft Office Suite (Outlook, Word, PowerPoint, and Excel).
- Basic medical knowledge, including ability to recognize social and medical risk factors, trauma symptoms and behaviors, and chronic disease conditions.
- Familiarity with the Healthcare industry.
- Knowledge of and compliance with HIPAA regulations.
Interpersonal Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Computer Skills
Has advanced basic computer job skills including logging on to systems, ability to communicate by email, ability to compose documents, enter database information, create presentations, download forms, and preserve/backup important data.
Reasoning Ability
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
