Responsibilities
- Evaluate residents’ functional abilities and therapy needs.
- Create and implement individualized treatment plans.
- Provide interventions focused on daily living skills, upper-extremity function, safety, cognition, and adaptive techniques.
- Maintain timely, compliant clinical documentation.
- Collaborate with residents, families, therapists, nursing, and other care partners.
- Current OT license or eligibility to practice in the applicable state.
- Graduate of an accredited Occupational Therapy program.
- Experience in skilled nursing, long-term care, or rehabilitation preferred.
- Strong clinical judgment, communication, and organizational skills.
