Responsibilities
- Conduct initial and ongoing assessments of residents to identify needs and develop individualized care plans
- Collaborate with healthcare team members to ensure comprehensive care and services are provided
- Maintain accurate and detailed records of assessments, care plans, and resident progress
- Develop and implement strategies to improve resident outcomes and reduce readmissions
- Participate in quality improvement initiatives and maintain compliance with regulatory guidelines
- Assist with resident admissions, discharges, and transfers
- Maintain a clean and organized workspace, adhering to infection control protocols
- Licensed Practical Nurse (LPN) with current licensure
- 1 years of experience in MDS
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
- Strong communication, interpersonal, and organizational skills
- Familiarity with MDS regulations and guidelines
- Basic knowledge of electronic medical records and Other software applications
