Develop and implement individualized care plans to address patients' and residents' social and emotional needs

Conduct social histories, identify risk factors, and develop strategies to mitigate risks
Provide crisis intervention and counseling services as needed
Maintain accurate and timely records of patient and resident interactions and treatment plans

Requirements:
Bachelor's or Master's degree in Social Work (MSW) from an accredited program
Possess a valid social work license in the state of New Jersey LCSW
Minimum 3 years of experience as a social worker in a healthcare setting
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment with multiple priorities
Familiarity with electronic medical records (EMRs) and Microsoft Office Suite
