Community Health Centers of America (CHCA) is seeking a highly organized and proactive SNF Clinic Lead (Medical Assistant) to oversee daily operations supporting our Skilled Nursing Facility (SNF) programs. This role serves as the central point of coordination between providers, facilities, credentialing, and internal teams, ensuring seamless onboarding, compliance, communication, and clinical workflow execution. This is an ideal role for someone who thrives in a fast-paced environment and can independently manage provider operations, documentation, and multi-facility coordination.
This will be a full time position based at our Fair Oaks office. This role offers the opportunity to make a meaningful impact in a dynamic and rewarding environment as a motivated individual with a passion for excellence and a commitment to providing top-tier support. The regular work schedule will be Monday - Friday days.

Key Responsibilities
Provider & Facility Coordination
- Serve as primary liaison between SNFs, providers, and internal teams
- Coordinate:
- Medical Director certification process
- EMR access across facilities (PCC, Matrix, etc.)
- Provider credentialing processes
- Psych service coordination
- QA/QAPI meeting scheduling and follow-up
- Manage SNF-related administrative workflows, including:
- Faxes, death certificates, and medical record requests
- Documentation tracking and provider follow-up
- Support providers with timely completion of required documentation
- Run facility admission reports and onboard new patients for providers
- Coordinate scheduling for:
- Covering MDs
- SNF visits
- Ensure providers have accurate, up-to-date patient information
- Support onboarding of new providers, including:
- Credentialing coordination
- EMR setup
- System access (TigerText, tokens, etc.)
- Maintain provider readiness across all assigned facilities
- Provide backup support for:
- Nocturnist schedules and call logs
- TCM scheduling
- Credentialing workflows (BVT, Sutter, Dignity)
- EMR management across systems
- Maintain and update key operational trackers, including:
- Post-Acute Provider List
- Delegation of Services Log
- Master Provider List
- Stipend tracking
- Track provider status changes (facility assignments, supervision agreements, malpractice updates)
- Respond to billing inquiries and support reconciliation
- Coordinate IT-related needs (provider devices, access)
- Process incoming mail and documentation
- Assist with recruiting efforts for MAs

Qualifications
Required
- Medical Assistant certification or equivalent experience
- 3 years in SNF, post-acute, or healthcare operations
- Strong understanding of provider workflows and facility operations
- Excellent organizational and communication skills
- Experience with SNF EMRs (PointClickCare, MatrixCare, etc.)
- Experience with credentialing and provider onboarding
- Familiarity with post-acute compliance and documentation workflows
