Job Summary
We are seeking a compassionate and dedicated Home Health Aide (HHA) to provide exceptional in-home care and support to individuals with diverse healthcare needs. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately
- Provide medication reminders and assist with medication administration following prescribed instructions while maintaining compliance with HIPAA regulations
- Support clients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice care, and working with individuals with developmental disabilities or disabilities
- Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure accurate patient records
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment
- Assist with medical procedures such as catheter care, wound care, or other treatments as directed by healthcare professionals
- Observe patient conditions closely and report any changes or concerns promptly to supervising healthcare providers
Skills
- Strong knowledge of medical terminology and patient care best practices
- Experience working in assisted living facilities, nursing homes, or home health environments
- Proficiency in using EMR/EHR systems for documentation and communication
- Excellent caregiving skills with a focus on compassion and patience
- Ability to work independently while following detailed care plans and protocols
- CPR certification and knowledge of emergency response procedures
- Familiarity with medical equipment such as Hoyer lifts and blood pressure monitors
- Effective communication skills to collaborate with clients, families, and healthcare teams
- Ability to handle sensitive information securely in compliance with HIPAA regulations
- Experience providing long-term care or home & community-based services is highly desirable
