Position Summary
The Clinical Nurse Manager (CNM) is responsible for the daily clinical operations and oversight of hospice nursing services at All Seasons Hospice. This role ensures delivery of high-quality, patient-centered care consistent with the organization’s mission, the CHAP standards. The CNM supervises field clinicians, promotes compliance, supports performance improvement activities, and ensures clinical excellence and regulatory adherence across all patient care services.
Essential Duties and Responsibilities
Clinical Oversight & Leadership
Provides direct supervision and support to Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and interdisciplinary team members.
Conducts regular case conferences to review patient plans of care, documentation, and adherence to hospice standards.
Oversees clinical workflows to ensure timely admissions, recertifications, and visits per plan of care.
Serves as a clinical resource and mentor, providing coaching, competency validation, and performance evaluations.
Assists in orientation and ongoing training of clinical staff to ensure compliance and professional growth.
Regulatory Compliance
Ensures all nursing services comply with CMS Conditions of Participation (42 CFR Part 418), CHAP standards, and Colorado Department of Public Health and Environment (CDPHE) regulations.
Participates in surveys, audits, and internal quality reviews, assisting in the development of corrective action plans as needed.
Monitors visit frequencies, documentation timeliness, and clinical outcomes to ensure compliance and optimal patient care.
Patient Care Coordination
Collaborates with physicians, interdisciplinary team (IDT) members, and families to ensure individualized, compassionate care.
Assists in managing complex cases, symptom management, and emergency interventions.
Reviews and approves clinical documentation for accuracy and completeness before submission.
Participates in on-call rotation as needed to support patient needs and staff coverage.
Quality and Performance Improvement
Contributes to the Quality Assessment and Performance Improvement (QAPI) program by tracking metrics such as symptom control, patient/family satisfaction, and clinical outcomes.
Identifies trends, develops improvement plans, and monitors progress toward goals.
Promotes evidence-based practices and continuous quality improvement across the team.
Administrative and Operational Duties
Assists in scheduling field staff and ensuring adequate coverage for patient visits.
Monitors productivity, caseload balance, and visit compliance reports.
Participates in budget planning and operational meetings as requested.
Represents All Seasons Hospice in community and professional settings as a clinical leader.
Qualifications
Education and Licensure
Current unrestricted Registered Nurse (RN) license in the State of Colorado required.
Bachelor of Science in Nursing (BSN) preferred; advanced hospice or leadership certification desirable (CHPN, CHPCA).
Experience
Minimum three (3) years of hospice experience required.
Minimum two (2) years of supervisory or management experience in a healthcare setting.
Strong understanding of hospice regulations, end-of-life care principles, and interdisciplinary coordination.
Skills and Competencies
Excellent leadership, communication, and interpersonal skills.
Ability to manage multiple priorities and foster teamwork.
Proficiency in EMR systems, documentation standards, and Microsoft Office Suite.
Compassionate, patient-centered approach with sound clinical judgment.
Physical and Working Conditions
Office and field-based role with frequent travel throughout the Denver Metro and surrounding areas.
Requires the ability to lift up to 25 pounds and perform physical assessments in home and facility settings.
Must have reliable transportation, valid Colorado driver’s license, and current auto insurance.
