Position Overview
The Physical Therapist (PT) will provide skilled physical therapy services to patients in the home health setting in accordance with physician orders and individualized care plans. Responsibilities include patient evaluations, treatment planning, ongoing assessments, documentation, patient education, and collaboration with interdisciplinary healthcare teams.
Responsibilities
Perform initial and ongoing patient evaluations
Develop and implement individualized treatment plans
Provide skilled physical therapy interventions in the home setting
Assist patients in improving mobility, strength, balance, safety, and functional independence
Provide gait training, transfer training, therapeutic exercises, range of motion activities, and muscle re-education
Educate patients and family members regarding treatment plans and home exercise programs
Evaluate home environments and recommend safety modifications or adaptive equipment
Maintain accurate and timely clinical documentation
Communicate patient progress and clinical changes to physicians and care team members
Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as required
Participate in care coordination and discharge planning
Follow all infection control, compliance, and documentation standards
Qualifications
Active Physical Therapist license in the state of practice
Graduate of an accredited Physical Therapy program
Minimum of one year of clinical experience preferred
Home health experience preferred but not required
Strong communication and time management skills
Ability to work independently and manage patient schedules efficiently
Preferred Experience
Candidates with experience in any of the following are encouraged to apply:
