Occupational Therapist (OT) | All Heart Home Care | Murfreesboro Area | Full-Time
All Heart Home Care is seeking a compassionate and skilled Occupational Therapist (Full-Time) to join our home health team in the Murfreesboro area. This role offers a consistent schedule while providing personalized, one-on-one care to patients in the comfort of their homes.
Key Responsibilities:
- Perform in-home evaluations to assess patient needs and develop individualized treatment plans
- Provide skilled occupational therapy services to improve daily living skills, strength, coordination, and independence
- Monitor patient progress and adjust treatment plans as appropriate
- Educate patients and caregivers on adaptive techniques, home safety, and therapeutic exercises
- Complete accurate and timely clinical documentation in compliance with company policies and state regulations
- Coordinate care with physicians, nurses, case managers, and other members of the healthcare team
- Travel to patient homes within assigned service areas
Qualifications:
- Degree in Occupational Therapy from an accredited program
- Active Occupational Therapist license in the state of Tennessee
- Current CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong clinical assessment and communication skills
- Ability to work independently and manage time effectively
- Previous home health experience preferred but not required
Work Schedule:
- Full-Time
- Consistent caseload based on patient volume and service area
