The Home Health Case Manager Position
We are looking for a full time RN for the position of Home Health Case Manager for our Columbia office. This person will be responsible to manage and coordinate appropriate home care services from admission to discharge. This position plans and coordinates the delivery of each assigned patient's plan of care in accordance with physician orders.

QUALIFICATIONS for Home Health Case Manager:
- Current, valid, and unencumbered Tennessee RN license or Compact State licensure
- Current BLS/CPR
- Current and valid driver license, transportation, and auto insurance
- Two or more years home health experience required
- One or more years EMR experience required
- Must be computer proficient
- Must be self-motivated and customer service oriented
- Must be located in the state of TN and able to travel to the office for care conference
