We are seeking a dedicated and skilled Physical Therapist (PT) to join our dynamic healthcare team. Monday-Friday 8am-4pm.
Position Type: Full Time Essential Duties & Responsibilities:
- Practice independently in the community as part of an interdisciplinary care team.
- Conduct comprehensive assessment of patients by determining patient needs, develops patients plan of care
- Manages assigned caseload; provides physical therapy services to patients as directed per PT plan of care.
- Documents all care provided and maintain records per All Care policies and standards.
- Participates in All Care team meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
- Ability to use, teach and assist patient with use durable medical equipment as a component of the plan of care.
Qualifications:
- Valid driver’s license is required.
- Active Massachusetts Physical Therapist (PT) license is required
- Previous experience as a physical therapist, preferably in a home health care, hospital, rehab or outpatient setting.
- Experience working with patients recovering from surgery, strokes, or managing chronic conditions is an advantage.
- Excellent communication and interpersonal skills to effectively educate and support patients and their families.
- Ability to work independently and manage patient care in a home setting.
- Proficiency in using electronic health record (EHR) systems for documentation and communications
