Essential Duties & Responsibilities
- Conduct comprehensive in-home assessments to determine hospice eligibility in accordance with Medicare and organizational guidelines
- Complete admissions, including clinical documentation, consents, and care planning
- Collaborate with physicians, case managers, and interdisciplinary team members to facilitate timely admissions
- Educate patients and families on hospice philosophy, services, and expectations
- Develop and initiate individualized plans of care
- Ensure accurate and timely documentation in the electronic medical record (EMR)
- Coordinate durable medical equipment (DME), medications, and initial services
- Provide emotional support and guidance to patients and caregivers during the admission process
- Maintain compliance with regulatory and agency standards
- Participate in on-call rotation as required
- Valid driver’s license and reliable transportation
- Active Massachusetts Registered Nurse (RN) license is required
- Minimum of 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong clinical assessment and symptom management skills
- Knowledge of end-of-life care practices
- Excellent communication and interpersonal skills
- Ability to work independently and manage a caseload
- Proficiency in using electronic health record (EHR) systems for documentation and communication
