Key Responsibilities
- Deliver comprehensive hospice nursing care in accordance with physician orders
- Perform patient assessments and develop, implement, and update individualized plans of care
- Manage pain and complex symptoms related to terminal illness
- Ensure accurate, timely documentation that meets Medicare and reimbursement guidelines
- Coordinate patient care with caregivers, physicians, and interdisciplinary team members
- Communicate effectively with patients and families, building trust and rapport at all levels
- Participate in Interdisciplinary Team (IDT) meetings and quality reviews
- Provide on-call coverage a minimum of one week per month, including evenings, weekends, and holidays as assigned
- Travel as required based on agency needs
- Active Registered Nurse (RN) license in Pennsylvania
- Minimum 2–3 years of clinical nursing experience
- Hospice or palliative care experience strongly preferred
- Strong clinical assessment and critical-thinking skills
- Excellent communication with patients, families, and care teams
- Valid driver’s license and reliable transportation
- Experience with Medicare hospice documentation
- Home health or hospice case management background
