Wound Care Coordinator

We are seeking a compassionate and dependable Wound Care Coordinator to join our team and play a key role in ensuring the health, comfort, and dignity of our residents. As a Wound Care Coordinator, you will provide hands-on care to residents, identify changes in condition, and evaluate care needs. You will also monitor overall condition and provide care in compliance with facility policies and procedures.

Essential Duties:

• Provide hands-on care to residents based on their Plan of Care.
• Identify changes in resident condition and evaluate care needs.
• Demonstrate strong assessment and documentation skills.
• Evaluate and participate in the development of the Resident Plan of Care.
• Monitor overall condition and provide care in compliance with facility policies and procedures.
• Provide primary skin care to residents under the medical direction of the attending physician or Director of Nursing.
• Identify, manage, and treat skin disorders, lesions, and decubitus ulcers.
• Ensure appropriate treatment and prevention for residents with decubitus ulcers, including daily inspection, turning, activity, nutrition, and cleanliness.
  
  
  

Requirements:

• Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state.
• At least 2 years of clinical experience in a long-term care, rehab, or skilled nursing setting.
• Prior experience providing wound care in a post-acute or long-term care facility.
