Home Care Assessment Nurse (RN)

As part of the nonprofit Givens Communities, Givens Home First & Givens Aldersgate supports older adults in living healthy, happy, and safe lives at home within our campuses, and across Charlotte, NC. At Givens Home First and Givens Aldersgate at Home, you’ll provide 1:1 care in a consistent setting, supporting our clients where they feel most at home.

What you'll do:

• Complete initial and 90-day home care assessments and supervisory visits
• Collaborate with caregivers, families, and leadership to ensure effective client care
• Maintain strong communication with leadership, clients, families, and caregivers
• Develop and update person-centered care plans
• Support the on-call needs of Home care, available for caregivers during evenings, nights, and weekends

What you'll need:

• Active North Carolina RN License
• Must have an active, unencumbered NC driver’s license
• Home care / home health experience preferred
• Strong communication and documentation skills

Compensation for this position is $33.50 - $38.50 per hour, based on experience.
