Our Care Managers are seasoned clinicians who provide both strategic direction and hands-on support. Deeply rooted in the community, we leverage a broad professional network to connect families with trusted resources, coordinate care, and facilitate meaningful conversations. Alder is seeking a dedicated, clinically licensed, and systems-savvy Care Manager to pioneer and grow our dedicated I/DD practice. This role is specifically designed for an experienced clinical professional, such as a state agency nurse, clinical social worker, or rehabilitative therapist, who is ready to transition away from high-volume public caseloads and return to high-touch, deeply personalized, person-centered care.

In this role, you will act as a strategic partner for individuals with developmental disabilities, autism spectrum disorders, and complex medical or mental health needs. You will manage a low-volume caseload, giving you the time, autonomy, and resources to build meaningful relationships with families and deliver the kind of thoughtful, person-centered care management that is often difficult to achieve in larger systems

Responsibilities

  • Holistic Care Management: Develop, implement, and oversee comprehensive, person-centered plans tailored to the unique goals, clinical needs, and long-term milestones of the individual.
  • Systems Navigation & Advocacy: Guide families through the complex ecosystem of public benefits, state agency eligibility processes, private resources, and insurance options, maximizing available funding streams.
  • Clinical Oversight & Coordination: Leverage your professional background to interpret complex medical or behavioral records, identify high-quality clinical providers, and ensure seamless continuity of care.
  • Transition & Lifespan Support: Provide expert guidance during critical milestones, including school-to-adult transitions, residential placement planning, employment changes, and future-care planning.
  • Interdisciplinary Collaboration: Act as the central liaison between families, medical professionals, educational entities, therapeutic teams, and community service vendors to ensure clear communication.
  • Clinical Accompaniment: Attend medical, therapeutic, and planning meetings alongside families to actively advocate for the individual’s clinical and human needs, ensuring follow-through from all parties.
  • Precise Documentation: Maintain accurate, timely, and thorough documentation of all care planning, clinical advocacy efforts, and family interactions.

Required Skills / Experiences

  • Licensure & Credentials: Must possess at least one of the following active Massachusetts credentials:
    • Master’s degree in Social Work (MSW) with an active LCSW or LICSW.
    • Master’s degree in Counseling or Psychology with an active LMHC.
    • Active license as a Registered Nurse (RN).
    • Active license as an Occupational Therapist (OT) or Physical Therapist (PT).
  • Experience: Minimum of 3 to 5 years of direct clinical or case management experience working with individuals with intellectual and developmental disabilities, preferably within a state agency ecosystem, regional clinical team, or specialized provider network.
  • Systems Expertise: Strong working knowledge of Massachusetts public and private benefit programs, including MassHealth, SSI/SSDI, Section 8, guardianship or its alternatives, and complex care funding streams.
  • Clinical & Advocacy Skills: Proven track record of practicing true person-centered thinking, translating clinical documentation into actionable care plans, and advocating successfully within complex institutional or medical systems.
  • Communication: Exceptional interpersonal, writing, and diplomatic skills, with the ability to build immediate trust with families experiencing high stress.

In accordance with the Massachusetts Pay Transparency Act, the expected pay range for this position is $60-$65 hourly. Actual compensation may vary based on factors such as licensure, years of experience, and other relevant qualifications.
