Position Summary: HCBS Coordinator
In this role, you will provide both direct support and team leadership, ensuring high-quality care and compliance with state regulations.
Key Responsibilities:
- Supervise and support a team of Direct Support Professionals (DSPs)
- Provide direct care and support as needed
- Assist individuals with daily living skills, personal care, health maintenance, and behavioral support
- Monitor health and safety and ensure accurate, timely documentation
- Collaborate with individuals served, family members, and other representatives to develop and implement service plans
- Maintain required documentation to ensure compliance with state standards and program requirements
Minimum Qualifications
- High school diploma or equivalent
- At least 18 years of age
- Valid Arizona driver’s license and insurable under AIRES policy
- Ability to pass background checks (Arizona Central Registry, Adult Protective Services)
- Ability to obtain and maintain a Level One Fingerprint Clearance Card
- Successful completion of required AIRES training
- Physical ability to assist with lifting/transferring up to 40 lbs
- Basic computer skills
- Strong communication and organizational skills
- Ability to build rapport with staff and individuals served
- Bilingual (English/Spanish) preferred
