Physician (primary Care)

Job description:

Provide high-quality, value-based primary care to Medicare patients in a collaborative outpatient setting, with a strong focus on preventive care, patient outcomes, and coordinated care. Qualified candidates may also be considered for a Medical Director role.

Responsibilities:

• Diagnose, treat, and manage acute and chronic conditions.
• Prescribe medications and maintain accurate electronic medical records.
• Collaborate with multidisciplinary care teams to achieve quality and patient care goals.
• Support clinical operations and help lead the care team.

Qualifications:

1. MD or DO with Board Certification/Eligibility in Internal or Family Medicine.
2. Active, unrestricted state medical license and DEA registration.
3. U.S. work authorization.
4. Preferred: Geriatrics experience, value-based care, outpatient primary care, and bilingual skills.
