SEEKING
We are looking for an individual to assist people affected by HIV living in Anderson, Greenville, Oconee, and Pickens Counties by providing access to a range of client-centered supportive services. The individual will assist clients and their families with personal and environmental difficulties that interfere with the ability to obtain maximum benefits from care.
Responsibilities
This position is responsible for preparing client histories, action plans, and service and medical reports; assisting clients obtain and maintain enrollment in benefit programs geared towards access to care and treatment; providing services planned to restore the client to optimum adherence to treatment, within the client’s capacity; helping clients (and their families) through individual or group conferences to understand, accept and follow medical recommendations; and, participating in planning for improving health services by interpreting social factors pertinent to development of client. The Medical Case Manager must utilize community resources; stay abreast of services available to clients, as well as the requirements of attaining/retaining those services; work in close collaboration with physicians, nurses, nurse educators, therapists, and other health care providers; and document all client-related activities according to agency and SC DHEC policy. Medical Case Managers manage a case load of 100 or more individuals.
The employee must abide by strict confidentiality requirements on and off duty. Note: This expectation extends beyond an employee’s employment period with AID Upstate.
ANNUAL SALARY
$46,700 - $48,500
Qualifications
The applicant must have a bachelor’s degree from an accredited 4-year college or university in a behavioral science and two years of related experience and/or training (or an equivalent combination of education and experience). The applicant must be able to work effectively with diverse populations. Additionally, the applicants must have knowledge of basic office equipment, including multi-line telephones, facsimile (fax) machines, copiers, computers, scanners, and printers, and experience with Microsoft Word, Outlook Email, basic computer database software, and Internet software. Bilingual is preferred but not required.
