How You’ll Make a Difference (Essential Functions)
The Clinical Educator is responsible for ongoing clinical education to Agrace staff and community partners on psychosocial issues in hospice and palliative care. The Clinical Educator works in collaboration with Quality, clinical leadership, direct care staff, and other clinical educators to support the development, implementation and instruction of clinical education for Agrace employees.

How You’ll Make a Difference (Essential Functions)
The Clinical Educator is responsible for ongoing clinical education to Agrace staff and community partners on psychosocial issues in hospice and palliative care. The Clinical Educator works in collaboration with Quality, clinical leadership, direct care staff, and other clinical educators to support the development, implementation and instruction of clinical education for Agrace employees.
Key Responsibilities
- Clinical Education for staff
- Design, develop, source, deliver, and evaluate learning opportunities that support staff clinical competency and skill development needs related to psychosocial aspects of care delivery.
- Facilitate, evaluate, and support the onboarding program for new clinical employees.
- Oversee the SW/SGC preceptor program.
- In collaboration with the Quality Team, use data to inform educational opportunities/training needs and develop plan to address knowledge gaps.
- Shadow clinical staff and audit electronic health records to ensure education is implemented.
- Coordinate, design and implement annual CEU education for counseling disciplines.
- Ensure clinical education meets the requirements identified by state/federal/Medicare Conditions of Participation.
- Resource for staff & partners
- Serve as a resource to counseling disciplines related to practice, boundaries and professional education development.
- Consult with Hospice Physicians and other internal and external clinical resources.
- Collaborate with clinical team leaders and staff to maintain and promote best practices.
- Clinical Education for Community
- Develop educational materials for patients, families and caregivers.
- Work with outreach liaisons to develop and present education that will both support partners and enhance knowledge in palliative and hospice care.
- Assist in the coordination and development of Agrace-sponsored conferences.
- Miscellaneous
- Participate in internal work groups, meetings and committees relative to position responsibilities
- Master’s in Social Work (MSW)
- Current, unrestricted Social Work License in state of employment
- Three or more years’ experience in health care preferably in geriatrics, hospice or palliative care with at least one-year paid medical social work experience
- One or more years’ experience in clinical education in a healthcare setting
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Ability to develop, maintain, and apply knowledge of hospice and end of life process
- Ability to stay abreast of current teaching and learning practices and clinical expertise
- Intermediate Microsoft Office skills
- Intermediate electronic health records skills; Epic preferred
- Ability to work effectively and cooperatively with others
- Ability to communicate effectively verbally and in writing
- Ability to be flexible in time and task to meet organizational needs
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions
