Summary/Objective
Executes and coordinates the daily activity program in the day center, mentors Activity Assistant, ensures safe, therapeutic, and engaging delivery aligned to care plans created by the Activity Supervisor.
Essential Duties/Responsibilities
- Creates weekly/monthly activity calendars that operationalize care plans (group and 1:1), balancing cognitive, physical, social, creative, and sensory domains.
- Facilitate group sessions (e.g., exercise, reminiscence, arts, music, brain fitness) and individualized engagements for ow participation or high -need participants.
- Coach Activity Assistant; assign tasks for coverage, monitor session quality and participant safety.
- Document Participant attendance, engagement levels, refusals, and observation, escalate notable changes to the Activity Supervisor.
- Set up /break down spaces, manage supplies/equipment.
- Implement quality improvement actions (e.g., enhancing engagement, reducing refusals) under direction of Activity Supervisor or with Interdisciplinary Team (IDT).
- Coordinate routine events/outings; ensure accessibility and risk mitigation are in place.
- Provide leadership and guidance to Activity Assistantl
- Develop written documents, procedure manuals, correspondence as directed by department supervisor.
- Communicate with all assigned staff on daily basis and report all areas of concern/issues/incidents to department supervisor immediately.
- Assist with safe ambulation/transfers within scope and training, maintain environmental safety and infection control.
- Serve as an example of agency Standard of Excellence by communicating to assigned staff and exemplifying agency vision and goals.
- Demonstrate professionalism in relationships with customers/co-workers/assigned staff/etc.; build rapport while refraining from inappropriately familiar personal relationships.
- Review and attest to accuracy of payroll records and submit for processing according to agency schedule.
Required Education and Experience
- Associate’s/bachelor’s degree in Recreation, Social Services or related field; experience in senior services preferred. High School Diploma or equivalent required.
- 2 years leading group programming; dementia-capable care experience strongly preferred.
- Strong group facilitation, de-escalation experience, and team leadership skills.
