Role Description This is a full-time, remote Registered Nurse role focused on providing hospice care and support to patients and families. The Registered Nurse will conduct remote assessments, care planning, and ongoing monitoring, collaborating with the interdisciplinary team to manage symptoms and coordinate services. Daily responsibilities include reviewing patient status, adjusting care plans in consultation with physicians and other team members, documenting care and communications, and providing education and emotional support to patients, families, and caregivers. The role involves frequent communication via phone and secure digital platforms, ensuring timely responses to patient needs and adherence to hospice standards and regulatory requirements. The Registered Nurse will also participate in team meetings, quality improvement activities, and continuous education to maintain best practices in hosp
ice care.Qualifications
- Active Registered Nurse (RN) license in the state of practice and eligibility to provide remote hospice care.
- Experience in hospice, palliative care, or related fields such as home health, oncology, or geriatric care.
- Strong clinical assessment, symptom management, and care planning skills, with the ability to work independently in a remote setting.
- Excellent communication and interpersonal skills for supporting patients, families, and caregivers with sensitivity and respect.
- Proficiency with electronic health records (EHR), telehealth tools, and basic computer applications.
- Ability to collaborate effectively with an interdisciplinary team and follow hospice regulations, policies, and best practices.
- Reliable internet connection and a suitable remote work environment; ability to manage time and priorities efficiently.
- Current BLS certification; additional hospice or palliative care certifications are preferred
