Position Summary
The Nurse Practitioner delivers direct primary and preventive medical care to patients and coordinates care with behavioral health, social work, and victim services staff to support Afya's integrated wraparound model.
Essential Duties and Responsibilities
- Provide comprehensive primary care services including history and physical examinations, diagnosis, treatment planning, and follow up care for patients of all ages served by Afya
- Order, perform, and interpret diagnostic and laboratory tests
- Prescribe medications and treatments consistent with Mississippi Board of Nursing scope of practice and any required collaborative or supervisory physician agreement
- Deliver care across all Afya programs, adjusting clinical approach to the needs of survivors, patients with substance use or behavioral health needs, and general primary care patients alike
- Maintain accurate and timely documentation in athenaOne consistent with Afya standards
- Participate in care coordination meetings with the LPN, CMA, Health Navigator, behavioral health, and social work staff to ensure wraparound continuity
- Support credentialing, quality assurance, and compliance activities required for Medicaid, Medicare, and CCO billing
- Maintain patient confidentiality and comply with HIPAA and all applicable state and federal regulations
- Participate in outreach or education events as needed to extend Afya's community health mission
Qualifications
- Active, unrestricted Mississippi license as a Nurse Practitioner
- Active national certification (e.g., ANCC or AANP) in an applicable population focus
- Current DEA registration or eligibility to obtain one
- Minimum of two years of clinical experience preferred
- Experience serving underserved, rural, or vulnerable populations preferred
Knowledge, Skills, and Abilities
· Strong clinical judgment and ability to work both independently and as part of an interdisciplinary wraparound team
· Cultural competency and trauma informed care skills, particularly with survivors of violence and trafficking
- Comfort working across a broad scope of primary care needs rather than a single specialty
- Proficiency with electronic medical records, athenaOne preferred
- Strong written and verbal communication skills
Working Conditions
Primarily clinic based with occasional local travel between Afya sites or community outreach locations. May require flexibility in scheduling to accommodate patient and program needs.
