- Assessing social and physical factors affecting discharge and connecting patients with community resources to meet post-discharge needs.
- Graduate of an accredited nursing school.
- Current Nevada Registered Nurse license.
- Knowledge of factors influencing discharge planning and community resources.
- Proficiency in computer programs including word processing, spreadsheets, and data management.
- Ability to provide age-appropriate care for diverse patient populations.
