We are seeking a licensed Occupational Therapist to provide personalized, one-on-one care to older adults in various settings such as private homes and senior living communities.

- Conducting evaluations and developing individualized treatment plans.
- Delivering evidence-based therapy to enhance mobility, independence, and overall wellness.
- Documenting care accurately within electronic medical records.
- Collaborating with a multidisciplinary team including PTs, SLPs, and fitness specialists.
- Educating patients, caregivers, and staff on safety and aging-in-place strategies.
- Providing virtual care options to increase flexibility and access.
- Valid Occupational Therapy license or eligibility in the practice state.
- Degree from an accredited Occupational Therapy program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Ability to exert moderate effort, including lifting up to 50 lbs occasionally.
- Performing patient transfers, repositioning, and standing/walking for extended periods.
- Driving to patient locations and wearing PPE when necessary.
