Role: Licensed Practical Nurse (LPN) providing in-home care for Pediatric and Adult patients, including those with GTube, Trach, and Vent needs.
Responsibilities: Utilize the nursing process to assess, plan, implement, and evaluate patient care. Deliver skilled nursing services according to prescribed orders, monitor signs of physiologic and psychosocial changes, and document diagnoses and patient data.
Qualifications: Valid LPN/LVN license in the state of practice, current TB or Chest X-Ray, BLS certification, and preferably at least one year of professional nursing experience.
