- Deliver patient-centered care aligned with individualized plans.
- Monitor and document vital signs, dressings, lab values, and patient responses.
- Communicate observations and changes in patient condition to the healthcare team.
- Administer and document medications per policy.
- Assist with patient orientation and ensure proper use of medical equipment.
- Perform additional duties as assigned.
- Completion of an approved LPN program recognized by the State Board of Nursing.
- At least one year of LPN experience or equivalent training through residency or program completion.
- Current LPN license recognized by the NCSBN Compact or state.
- Optionally, progression to RN licensure is supported, with notifications required upon licensure attainment.
