Care Manager MSW

Summary

This role involves coordinating care for complex patients from pre-admission to post-discharge, promoting continuity of care, psychosocial outcomes, patient advocacy, and resource management under the supervision of the Care Management Manager.

Qualifications

• Master's Degree in Social Work or related field
• Washington Clinical Independent Social Worker Associate License
• Minimum 3 years of social services experience, including 2 years in acute care or care coordination
  
  

Preferred:

• Accredited Case Manager (ACM) or Certified Case Manager (CCM)
  
  

The position is on-site, full-time, with a pay range of $36.00 - $56.84 hourly. Key duties include care coordination, patient advocacy, and resource management, supporting a patient-centered, inclusive work environment focused on high-quality healthcare delivery.
