This opportunity is open to the entire state of CT, as we provide services to the whole state, but you will be paired with an individual within your geograhpic region for ease of service.
The volunteer selection process at Affinity/Continuum begins with an application and an interview, where potential volunteers may express their personal goals, availability, interests and talents. Volunteers are carefully screened through a criminal background check, motor vehicle report, and personal references to ensure our patients are under the best care possible.
Responsibilities
- Offer companionship and emotional support to hospice patients, fostering a sense of comfort and connection.
- Assist with activities of daily living (ADLs), including grooming, errands, and light housekeeping tasks such as cleaning and laundry.
- Ensuring safety and adherence to HIPAA regulations.
- Help with meal preparation
- Provide transportation assistance for appointments or outings within the community when needed.
- Maintain detailed documentation of patient interactions and observations to support ongoing care efforts.
- Assist with office management
Qualifications
- MUST HAVE RELIABLE MEANS OF TRANSPORTATION
- Strong communication skills
- Compassionate attitude with a genuine desire to support patients through hospice care or home health services
- Must be reliable, adaptable, and able to work independently or as part of a team dedicated to providing exceptional hospice care
- Prior experience in home & community care, assisted living, nursing homes, or long-term care environments is highly desirable
Work Location: In person
