Wraparound Facilitator Description

The Wraparound Facilitator will engage high-need youth and their families in developing, implementing, and monitoring an intensive, individualized care plan. This position helps the youth and families to identify a support team who will collaboratively design a plan based upon family needs, strengths, and goals. The Wraparound Facilitator will support the team as they implement the plan and monitor its success.

Job Summary: To facilitate Wraparound team meetings including the development, implementation and monitoring of individualized care plans for high-need youth.

Wraparound Facilitator Role:

  • Coordinates and facilitates Wraparound team meetings
  • Provides leadership in the development, implementation and evaluation of individualized plans using the phases and activities of Wraparound
  • Develops trust and mutual respect with youth and their teams
  • Assists teams to identify needs and create a shared vision
  • Assists families in identifying, recruiting and engaging natural supports
  • Enables teams to take ownership of the Wraparound process
  • Supports clients in identifying their strengths and building upon them
  • Develops the problem-solving skills and self-efficacy of the youth and team members
  • Identifies barriers to full participation in the Wraparound process and develops strategies to overcome them
  • Ensures the Wraparound processes is completed with high fidelity to the model and tracks progress toward goals
  • Provides support and advocacy for Wraparound teams
  • Prepares summary documents and final reports
  • Ensures that all required confidentiality and other compliance documents are current for each youth
  • Educates families, providers and the community about the Wraparound process
  • Contributes to and supports a positive, team-oriented work environment
  • Completes documentation of services and treatment plans consistent with clinical and administrative policies and procedures
  • Promotes and demonstrates APP’s mission, vision and values through both behavior and job performance on a day to day basis
  • Effectively interacts and communicates with other APP staff/clients/customers/partners/etc.
  • Regular and consistent attendance is required to perform other essential functions of the job
  • Other duties as assigned
  • Work in relationship with local schools and school liaison’s.
  • Communicate with your client as need arises
  • Attend trainings as offered by the agency
  • Be at least 21 years’ old
  • Adhere to all program policies, procedures and follow the Social Workers and Counselors code of ethics
  • Be dependable and consistent in meeting with client’s and agency required meetings.
  • Communicate regularly with program director, staff, submit activity reports and Monthly reports (by the 1 of the month), and take constructive feedback regarding mentoring, activities and relationship with mentee and families
  • Have access to an automobile, auto insurance, and a good driving record
  • Pass a criminal background check
  • Not use illicit drugs
  • Not use alcohol or controlled substances in an inappropriate manner
  • Not be currently in treatment for substance abuse or recently released
  • Not smoke or use tobacco products during activities or promote their use
  • Use technology appropriately, and not be distracted from client by personal technology

Parent Support Specialist Role:

GENERAL DESCRIPTION:

The Parent Support Service Staff will provide in-home Education Services aimed at preventing potential or secondary abuse and neglect for an “at risk” population of parents or care-givers who are residents of Ashland County, Ohio. Parents participating in the programming will be referred by a variety of referral sources through local agencies or self-referral. The Parent Educator will support the parents through education and service coordination. The Parent Educator will also provide positive programming for groups of parents in the community that is open to all parents of Ashland County.

QUALIFICATIONS:

The Parent Support facilitator should have the following skills and experience:

  • Bachelor’s degree in social service-related field, education or equivalent combination of education and experience
  • The ability to communicate with, nurture, educate and empower parents or care-givers to be effective in their roles
  • The ability to work cooperatively with different types of personalities
  • Knowledge and understanding of issues and dynamics within families in crisis relating to child abuse and neglect
  • Commitment to strengthening and supporting families
  • Ability to read, write and speak English; able to help families develop case goals and plans
  • Ability to work independently but with strong teamwork commitment.
  • Ability to use the computer for case notes, entry into the database and periodic reporting.
  • Good interpersonal, communication and organizational skills.
  • High ethical standards. No criminal record or history of founded child or adult abuse/neglect.
  • Valid driver’s license, reliable vehicle and car insurance
  • Evenings and weekend availability

ACCOUNTABILITY:

The Parent Educator reports directly to Parent Support Team Lead and the executive director, who is responsible for his/her performance evaluations and board interaction

RESPONSIBILITIES:

  • Schedule, conduct and document regular home/office visitation sessions with participants (parents and/or caregivers) and their children to deliver a curriculum designed to teach basic parenting skills and child development. This may include evenings and weekends.
  • Provide group and community education related to the program.
  • Promote community awareness of programs through presentations to community, church, civic and school groups and through use of local media. Collaborate with community partner agencies to access services for participating families. Complete required paperwork, documentation and grant reports for home visitation and school based programs. Attend any required meetings planned by grant giving agencies. Coordinate activities of volunteers who may be available to provide services for the program preparation Evaluate program effectiveness.
  • Review new cases and follow up with case workers / managers
  • Prepare and distribute assignment documentation
  • Help develop initial case plans and ongoing strategies for advocacy
  • Maintain case files in office
  • Attend court hearings when subpoenaed or requested by client
  • Attend staff meetings
  • Attend in-service trainings and assist with coordination if requested
  • Provide office coverage as assigned by the executive director
  • Complete work time sheets monthly
  • Attend conferences/seminars/meetings as requested by the executive director
  • Other duties as may be assigned by the executive director or the chair of the board

Job Types: Full-time, Part-time

Pay: $16.00 - $17.00 per hour

Expected hours: 20 – 30 per week
