What are the responsibilities and job description for the Service Coordinator position at Advocated Centered Services?
Advocate Centered Services is looking for an individual to join our team! Individual should live in Dauphin County, York, Lancaster or Lebanon PA and be able and willing to travel to the surrounding counties to see participants. This is full time Service Coordinator employment opportunity; Monday thru Friday 40 hour a week. Position is 75% work from home and 25% travel to participant homes (located within counties mentioned), no on call. Persons must be self-motivated, have reliable transportation, reliable internet, and the willingness to travel to participants homes. Familiarity with PA Medicaid is preferred, but not mandatory. Applicants musthave a bachelor's degree in social sciences or have 3 years relatable experience. Additional details include:
- Paid Remote Training (with the exception of the 1st day you will travel to the corporate office located in Dubois, PA)
- Weekly online continued training
- Government Holidays
- 2 sick days every 6 months (immediately)
- 5 vacation days (after a year) with and additional day on yearly basis
- Health insurance is offered after 60-days (probationary period)
- 180 days probation, then possible raise $0.50 per mile Transportation Reimbursement for all miles traveled to visit participants
- Case load of 50-60 participants
