Regional MDS Nurse

AdviniaCare is seeking a highly experienced and skilled Regional MDS Nurse for our Rhode Island facilities to join our team! As a Regional MDS Nurse, you will be responsible for overseeing MDS nurses and departments at multiple facilities in Rhode Island, tracking and trending CMI, and ensuring compliance with PDPM regulations. Responsibilities

• Oversee MDS nurses and departments at each facility in the state of Rhode Island
• Track and trend CMI to ensure compliance with regulatory requirements
• Ensure accurate and timely completion of MDS assessments and care plans
• Provide guidance and support to MDS nurses and departments to improve care outcomes
• Collaborate with multi-disciplinary teams to develop and implement care plans
• Stay up-to-date with changes in PDPM regulations and ensure compliance
  
  
  

Requirements

• Full-time position with 40 hours/week commitment
• Regional MDS Nurse with experience in multi-facility oversight
• 3-5 years of MDS experience
• RN preferred, but LPN with experience considered
• RAC-CT preferred
• Strong understanding of the MDS process and ability to complete MDS assessments and care plans when needed
• Experience with PDPM regulations a must
