The role you'll contribute:

Responsible for coordinating and directing home care for patients based on individual needs. This position independently manages the home health patient population, utilizing advanced assessment, teaching, and decision-making skills. This role ensures appropriate referrals to other services, regular interdisciplinary conferencing, and completion of necessary documentation. Other responsibilities include educating community members and patients, evaluating the effectiveness of the home care plan, and monitoring patient and family responses to achieve goals and outcomes.

• Evaluates the home environment for safety, infection control, and community resource needs.
• Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician.
• Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities.
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in various home situations.
• Uses motivational interviewing and health coaching techniques to engage stakeholders in care management.
• Informs the physician, clinical manager, and healthcare team of changes in the patients condition and needs.

The expertise and experiences you'll need to succeed:

Minimum qualifications:

• Associate's of Nursing
• 1 year relevant clinical nursing experience
• Registered Nurse State Licensure and/or Compact State Licensure In Florida
• Active American Heart Association BLS
• Valid in state Drivers License with current car insurance

Preferred qualifications:

• Bachelor's of Nursing
• Cert for Oasis Specialist-Clinical (COSC)
