Job Description: Responsible for providing one-on-one skilled nursing care for assigned patients on hospice care in private homes or in skilled nursing facilities/assisted living facilities, in compliance with state and federal regulatory and licensure standards. This position completes training to be available to work in the In Patient Unit (IPU) and in the field as a flex nurse. Additionally, this role ensures adherence to hospice care protocols and maintains accurate patient records. In addition, this position collaborates with healthcare providers to deliver comprehensive care. Ensures all activities adhere to relevant guidelines and standards.

• Provides skilled nursing care to hospice patients requiring crisis care in various settings.
• Assesses patient conditions and symptoms necessitating crisis care.
• Administers medications and treatments as prescribed.
• Monitors and manages symptoms to ensure patient comfort.
• Communicates with the healthcare team regarding patient status and care plans.

Knowledge, Skills, and Abilities:

• Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
• Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee.
• Knowledge of electronic medical record.
• Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form.
• Ability to follow standard precaution guidelines for infection control, including when providing patient care.

Education:

• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]

Work Experience:

• 2 experience as a registered nurse [Required]
• Home health and/or acute care nursing experience [Preferred]

Licenses and Certifications:

• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
• Driver's License (DL) [Required]
• Certified Hospice and Palliative Nurse (CHPN) [Preferred]

Pay: $29.21 - $46.73 per hour
