The role you'll contribute:
Responsible for implementing the treatment plan developed and supervised by the Physical Therapist. This position provides therapeutic interventions to improve patients' functional abilities. This role adheres to department standards and ensures compliance with all applicable local, state, and federal regulations, as well as accrediting bodies. In addition, this role documents patient progress and collaborates with the healthcare team.

- Implements treatment plans established by an Physical Therapist utilizing recommended techniques, adhering to insurance guidelines and patient needs
- Documents interventions and patient progress accurately, completely, and timely, using measurable and objective terminology in line with standards of practice, insurance guidelines, and department/hospital policies and procedures
- Maintains productivity standards as defined by the department and adjusts work hours to meet the needs of patients
- Educates patients, family members, and caregivers on identified needs, ensuring understanding
- Monitors patient response to treatment and communicates with the Physical Therapist for necessary adjustments
- Maintains open communication with the case manager, primary therapist, and other healthcare professionals to facilitate interdisciplinary continuity of care and appropriate discharge planning
The expertise and experiences you'll need to succeed:
Minimum qualifications:
- Associates degree in Physical Therapy Assistant
- Physical Therapy Assistant (LPTA) State Licensure and/or Compact State Licensure
- Active American Heart Association BLS
- Valid in state Drivers License with current car insurance
Preferred qualifications:
- 1 year home health experience
