JOB DESCRIPTION:
We are seeking an experienced and dedicated Nurse Practitioner to join our Advanced Wound Care team. In this role, you will assess, diagnose, and manage patients with acute and chronic wounds, providing expert care and developing individualized treatment plans. You will collaborate closely with our interdisciplinary clinical team to ensure optimal patient outcomes, perform wound assessments, and oversee advanced wound care therapies. In addition, you will be responsible for performing grafting procedures and making regular follow-up calls to monitor patient progress, adjusting care plans as needed to support healing. The ideal candidate will have a strong background in wound care, excellent clinical skills, and a passion for delivering high-quality, compassionate care to patients.
KEY RESPONSIBILITIES:
- Assessing patients in their homes, cleaning and dressing wounds, and applying grafts.
- Educating patients and their families on proper wound care.
- Completing patient charts accurately and in a timely manner, ensuring compliance with clinical guidelines and HIPAA regulations.
- Developing and adjusting individualized treatment plans based on patient assessments and progress.
- Collaborating with the interdisciplinary team to monitor and optimize patient outcomes. Managing follow-up care to ensure continuity and effective wound healing.
- Providing clinical supervision and guidance to nursing staff on wound care best practices.
- Staying current with advancements in wound care and incorporating new treatments into practice.
QUALIFICATIONS:
- Master's or Doctorate degree in Nursing.
- Valid Nurse Practitioner (NP) license in Texas.
- Valid driver's license and reliable transportation for patient visits.
- Ability to assess situations for safety and ensure a secure environment for both patients and staff.
- Experience with maintaining up-to-date patient charts, tracking progress, and ensuring compliance with company standards and healthcare regulations.
- Ability to perform physical tasks, including lifting, positioning, and standing for extended periods.
- Excellent written and verbal communication skills for documentation and teamwork.
- Knowledge of state and federal healthcare regulations, HIPAA, and safety standards.
- Compassionate and empathetic approach to patient care.

Pay: $85.00 - $100.00 per hour
