Outpatient, and other outpatient services that provide individuals experiencing

homelessness and re-entry with access to vital mental health and addiction services. Through compassionate care, therapy, and support, we aim to empower these individuals to regain stability, independence, and hope, ultimately breaking the cycle of homelessness and addictions by improving their overall well-being.

Duties

• Conduct thorough assessments and evaluations of clients to determine mental health needs and diagnoses.
• Develop individualized treatment plans tailored to each client's unique goals and needs.
• Provide individual therapy sessions, group therapy, and family therapy as needed, employing evidence-based therapeutic approaches (e.g., Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Solution-Focused Therapy, etc.).
• Maintain accurate and up-to-date client records in compliance with all regulatory and ethical standards.
• Monitor client progress and adjust treatment plans accordingly.
• Provide crisis intervention when necessary, including de-escalating situations and offering immediate support to clients in distress.
• Collaborate with other healthcare professionals (doctors, nurses, social workers, etc.) to ensure holistic care for clients.
• Educate clients and their families on coping strategies, mental health management, and treatment goals.
• Provide referrals for additional services, such as psychiatric care or substance abuse treatment, when necessary.
• Participate in team meetings, case discussions, and ongoing training and development to stay current with best practices and treatment modalities.
• Maintain confidentiality and adhere to ethical guidelines, including HIPAA regulations.

Qualifications

• Masters preferred will accept bachelor’s degree in social work, psychology, social sciences, or counseling- related field.
• Experience in facilitating therapeutic groups.
• Strong interpersonal and communication skills.
• Ability to create a safe and inclusive group environment.
• Commitment to promoting a culture of diversity, equity, and inclusion
• Strong communication skills, both verbal and written.
• Ability to work collaboratively within a multidisciplinary team.
• Compassionate demeanor with a genuine interest in helping others improve their mental health.
• Previous experience in a mental health or healthcare setting is advantageous but not required.
