Primary Responsibilities and Duties
- Administer medications and treatments as prescribed by physicians and in accordance with home health care protocols.
- Perform routine and specialized nursing tasks, including wound care, catheter insertion, and monitoring vital signs.
- Assist in developing and implementing individualized care plans in collaboration with patients, families, and the healthcare team.
- Monitor and document patient progress, responses to treatment, and changes in condition, promptly reporting significant findings.
- Educate and support patients and families on disease management, medication use, symptom monitoring, and home care techniques.
- Assist patients with mobility, transfers, and activities of daily living as needed, promoting safety, comfort, and independence.
- Maintain accurate, timely, and up-to-date patient records in compliance with regulatory and organizational standards.
- Collaborate with LPNs, physicians, and other healthcare professionals to provide coordinated and comprehensive care.
- Respond promptly to patient needs and changes in condition, providing appropriate interventions and support.
- Follow infection control and safety procedures to protect patients, families, and staff.
- Perform other duties as assigned.
Job Qualifications
- Valid Registered Nurse (RN) license in the state where work is performed.
- Two or more years of nursing experience; home health, hospice, palliative care, long-term care, or a related field is preferred.
- Strong clinical skills and knowledge of nursing principles, procedures, and best practices.
- Excellent communication and interpersonal skills, with the ability to build trust and interact effectively with patients, families, caregivers, and healthcare professionals.
- Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
- Understanding of regulatory requirements related to home health and nursing practice.
- A compassionate and caring demeanor with the ability to build meaningful rapport with patients and families.
Physical Requirements
- Ability to lift and transfer patients, with or without assistance, up to 50 pounds.
- Ability to bend, stand, walk, and climb stairs for extended periods.
- Strength to handle medical equipment and assist patients with mobility and physical activities.
- Visual acuity and dexterity to read medical records, administer injections, and handle delicate medical instruments.
- Ability to respond swiftly and efficiently to emergency situations.
- Ability to work in a variety of home environments, including spaces with stairs, tight areas, or uneven surfaces.
- Regular attendance and punctuality in accordance with the schedule designated by the department supervisor or manager.
- Ability to accept feedback and respond calmly and effectively in high-stress situations.
- Ability to use the abdominal and lower back muscles to support part of the body repeatedly or continuously over time without fatigue.
- Ability to keep the hand and arm steady while moving the arm or holding the hand and arm in one position.
