What are the responsibilities and job description for the LPN Case Manager position at Adobe Care And Wellness Arizona LLC?
We are looking for a dynamic licensed practical nurse with a current Arizona license who wants to be a part of an outstanding case management team.

Job Essentials Include:
- Provide Case Management field-based services to assigned client population in Sierra Vista.
- Complete assigned assessments and develop appropriate care plans
- Provide care coordination to alleviate identified needs related to behavioral, physical and social issues
- Focus on addressing issues that will decrease medical costs and increase quality of life
- Interact with peers in a collegial manner, supporting efforts to improve outcomes
- Participate in on-call rotation for after-hours services as needed
You will manage health problems, connect with community services, identify dietary concerns, conduct home safety evaluations, educate on specific illnesses, treatments, and safe medicine use.
You must have:
- Office 365 experience along with some generic Health Management System experience.
- Excellent verbal and written communication skills
- Basic knowledge regarding HIM coding standards
- Analytic skills necessary to accurately assess patient medical records
- Ability to be flexible and adjust to workload/assignment changes, interruptions and stress
