Requirements
- ABOS board certified or truly board eligible in Orthopedic Sports Medicine; must obtain board certification within five years — no exceptions
- Valid Illinois and Iowa medical licenses or active IMLC with willingness to obtain state licenses
- Strong arthroscopic and open sports surgery skills required; shoulder, knee, and hip experience preferred
- Current, unrestricted federal and state authority to prescribe and administer medications appropriate to the specialty
- Active DEA registration
- BLS certification with ongoing recertification
- Clean malpractice and licensure history highly preferred
- No visa support for J-1 or H-1B candidates
- Submit a Self-Query NPDB report pulled within 30 days of presentation
