- Board Certified in OB/GYN (required)
- Active California medical license (or in process) (required)
- DEA registration must be obtained prior to start (required; no exemptions)
- Clean malpractice and license history (highly preferred)
- Most recent NPDB Self Query Report (within 30 days of presentation) (required; no exemptions)
