We are seeking a detail-oriented and reliable individual to serve as Home Health Orders Liaison. This role is responsible for visiting hospitals, physician offices, and other healthcare facilities to obtain timely signatures on home health orders, plan of care documents, and other required clinical paperwork. The ideal candidate will serve as a professional representative of our agency and ensure seamless coordination between referral sources and our clinical team.
Key Responsibilities
- Travel to hospitals, physician offices, and other healthcare providers to deliver and retrieve home health documentation.
- Obtain timely and accurate signatures on physician orders, including plans of care, re-certifications, and medication changes.
- Communicate effectively with physicians, office staff, discharge planners, and case managers to ensure documentation compliance and timely processing.
- Maintain accurate records of all visits, documents delivered/collected, and any pending or follow-up needs.
- Identify and escalate any issues related to delayed or unsigned orders.
- Collaborate with internal departments to ensure coordination of patient care and documentation timelines.
- Maintain confidentiality and comply with all HIPAA regulations and company policies.
- High school diploma or equivalent required; associate or bachelor’s degree preferred.
- Experience in a healthcare setting or home health agency preferred.
- Excellent communication and interpersonal skills.
- Professional appearance and demeanor.
- Reliable transportation, valid driver’s license, and current auto insurance required.
- Strong attention to detail and time management skills.
- Ability to work independently with minimal supervision.
- Familiarity with electronic health records (EHR) systems and basic computer skills a plus.

- Occasional lifting and carrying of documents and materials (up to 25 lbs).
