The Adams County Health Department is seeking a dedicated and outgoing Community Health Worker (CHW) to join our team. This position has the opportunity to be either full or part-time. This professional serves as a vital bridge between the health department and the community, ensuring residents have access to the resources necessary for healthy living. Operating as a frontline advocate, the CHW provides outreach, education, and informal counseling to support a wide range of public health initiatives.
Key Responsibilities
· Leads outreach efforts to promote the importance of preventative screenings, assisting clients in navigating the healthcare system to secure appointments
· Assists families in accessing services and navigating nutritional benefits (SNAP and WIC) to improve food security and healthy development
· Educates the community on the significance of dental hygiene and connects residents with local oral health resources and clinical services
· Represents the department at local events, health fairs and community meetings to build public trust and increase awareness of available programs
· Helps individuals overcome barriers to care by providing direct referrals for social services, transportation or language assistance
Qualifications
· Knowledge of the Adams County community and its unique resources
· Excellent communication skills, with a demonstrated ability to explain health information in clear, relatable terms
· Strong organizational skills for tracking client progress and documenting outreach data
· High school diploma or equivalent
